Dog License Application Online Submission

Dog licenses are required for all dogs over 5 months of age in the Town of Merton.

If your dog is not licensed prior to April 1st 2023, a $5.00 late penalty fee per dog will be added to the regular license fee. A copy of the rabies certificate is required and needs to be attached to this application. The certificate should show the name of vet clinic & phone number and vaccine expiration date, as well as the vaccine manufacturer and serial number.

Please be sure to select "Dog License" under Payment Type and enter the appropriate fees (including any late fees for paying after April 1st) under Payment Amount. Fees apply for using this service.

Note that you can pay for various fees using this link, however, please ensure that you complete ALL applicable applications online (dog license application, compost pass application, and operator's license application) for those fees that you are paying.

To mail application: Send a check with a printed copy of this application and proof of rabies certificate  to: Town of Merton, PO Box 128 North Lake, WI 53064.

After application and payment are received: The Town of Merton will mail the tags and a receipt to the resident. Please allow for mail time when applying online. THANK YOU.

Late Fees (Applied after March 31)
If licensing your dog after April 1st, a $5 per dog late fee applies. There are two exceptions to the late fee: 1. If you need to wait to vaccinate your dog until they are 5 months old and that occurs after April 1st, there is no late fee. 2. If you acquired your dog after April 1st, there is no late fee.
Explain why a late fee may not apply to (all) dog(s).
